5V5 HOLIDAY CLASSIC REQUIRED FORMS

Complete the online tournament roster

  • »  Preliminary tournament roster is due December 18. Roster changes may occur until December 26 at 4 pm
  • »  Final rosters are then printed for Final Team Check-in
  • »  Maximum roster size is 18
  • »  Addresses required. Player addresses are linked to the NSC Liability Waiver.
  • »  Team manager/coach cell phone number required

Direct parents/adult players to complete the online NSC Liability Waiver

  • »  Provide parents/guardians or adult players with the tournament ID number for this task.
  • »  You may monitor progress by selecting View Roster from the online roster menu. Each time a waiver is completed, an X will appear in the waiver (WV) column to the left of the player's name.

Lodging

  • »  If your team is staying in a hotel, view our Official Host Hotels for a list of required hotels, rates and amenities.
  • »  Email jeen@nscsports.org with the name, city and phone number of your hotel as back-up emergency contact. Be sure to include your tournament ID number in the subject line.

Final Team Check-in 

  • »  Location: NSC Sports Hall, stage between fields A and B.
  • »  Representative must check in team one hour prior to team's first game
  • »  Proof of age required for all youth players. Player pass, copy of birth certificate or driver's license accepted.
